Application for Admission

We welcome new and returning students. Please read this page to be aware of the laws and requirements of Holy Name School and the state of Massachusetts.

Registration Requirements

A child entering kindergarten must be five years old by September 1. At Holy Name School, we require the following documentation for students entering any grade:

  • Birth certificate
  • Immunization record
  • Previous year’s report card, if applicable
  • Baptismal certificate (Catholics only)
  • A non-refundable registration fee per family after the child is accepted into the school

Immunizations

In order to better understand your child’s health status and to meet the Massachusetts Department of Public Health regulations, we require information about your child’s past and present health history and immunization status. You must complete this information before we can allow your child to enter the Holy Name School.

In accordance with Massachusetts state law, we require students to be in compliance with the required immunizations. New students should also provide a completed physical examination form:

  • DPT/DTaP:
    • Preschool: 4 or more doses
    • Kindergarten: 5 or more doses
    • Grades 1–6: 5 or more doses
    • Grade 7: 1 dose of Tdap
  • OPV/IPV (polio):
    • Preschool: 3 or more doses
    • Kindergarten: 4 doses
    • Grades 1–7: 4 doses
  • MMR (measles, mumps, rubella/German measles):
    • Preschool: 1 dose
    • Kindergarten–Grade 7: 2 doses
  • Hepatitis B:
    • Preschool–Grade 7: 3 doses
  • Hib (haemophilus influenza type B):
    • Preschool: 1–4 doses (depends on age of child at initial dose)
  • Varicella (chicken pox):
    • Preschool: 1 dose
    • Kindergarten: 2 doses
    • Grades 1–6: 2 doses
    • Grade 7: 2 doses
  • Meningococcal:
    • Grade 7: 1 dose
  • Tb (tuberculosis screening):
    • Preschool–7: TB Risk Assessment (one documented risk assessment from anytime since birth)
  • Lead Screening:
    • Preschool or Kindergarten entry: Documentation of blood lead screening.

These requirements allow for medical reasons and religious beliefs. If your child is exempt from immunizations, we may remove him/her from school during an outbreak.

Tuition

It's easy to make your tuition payments is through FACTS Tuition Management at (800) 624-7092, or you may make payments on their website. For the upcoming school year (10, 11, or 12-month cycle), tuition payments should begin in July.

2022–2023 School Year:

  • Pre-Kindergarten Half Day: $4,250 plus $250.00 Scholastic/Technology Fee Total: $4,500.00
  • Pre-Kindergarten Full Day: $5,250.00 plus $250.00 Scholastic/Technology Fee Total: $5,500.00
  • Kindergarten–Grade 8: $4,550.00 plus $250.00 Scholastic/Technology
    Fee Total: $4,800.00

2023–2024 School Year:

  • Pre-Kindergarten Half Day: 8:00 to 11:30 a.m. - $4,350 base tuition plus $250.00 Scholastic/Technical Fee
    Fee Total: $4,600.00
  • Pre-Kindergarten Full Day: 8:00 a.m. to 2:20 p.m. - $5,350 base tuition plus $250.00 Scholastic/Technical Fee
    Fee Total: $5,600.00
  • Kindergarten–Grade 8: 8:00 a.m. to 2:20 p.m. - $4,650 base tuition plus $250.00 Scholastic/Technical Fee
    Fee Total: $4,900.00

Scholastic/Technology Fee

There is a scholastic/technology fee of $250 for each student. The payment of this fee reserves the registration placement for the child.
